1. PRE-MEMBERSHIP ORIENTATION SEMINAR (PMOS)

  • Every Thursday @ 8:30 AM SAMELCO I – HQ
  • Every Tuesday  @ 10:00 AM Gandara Service Station

2. Application for Electric Service (MSD Form 1)

3. Membership Application (MSD Form 2)

4. Proof of Right to Occupy the Land

  • if owned:
    • OCT/TCT or Tax Declaration (photocopy)
  • if title/tax dec is not registered to applicant’s name:
    • Private Land (either of the following)
      • OCT/TCT  (photocopy)
      • Tax Declaration (photocopy)
    • Proof of Transfer of Rights
      • Deed of Sale
      • Extra-judicial Settlement, Death Certificate and Birth Certificate
      • Lease Contract
      • Usufruct
    • Proof of Consent
      • Naturalized written consent of registered owner (MSD Form 3)
      • Waiver & Consent to be disconnected (MSD Form 4)
    • Public Land
      • Barangay Certification or Assessor’s Certification
      • Written Consent from the LGU allowing the applicant to have electric connection
      • Waiver & Consent to be disconnected (MSD Form 4)

5. One (1) Valid Identification Card

6. Schematic Diagram (Registered Electician or BFP)

7. Fire Safety Clearance (BFP)

8.  Electrical Plan (signed by PEE)

9. Electrical Permit (LGU Building Official)

10. Barangay Certification

11. INSPECTION (if the structure is ready for energization)
